It is our mission at Every Moment Matters Agency to give all individuals equal opportunity to fulfill their fullest potentials. Despite disability or functional limitations, they can be capable of community integration and becoming a contributing member of their families and their neighborhoods.
Our programs and services are geared towards helping these individuals achieve their goals. With personalized care, social services and community service coordination, they can avoid institutional care – instead, they can continue living with their families at home.
As much as possible, we will address their disability care needs while also giving family members relief from non-stop caregiving responsibilities. By sharing the caregiving responsibilities with a professional care provider, it improves lives not just for the individual with disability but also for family members.
Every Moment Matters Agency can facilitate state-funded programs for individuals with developmental disabilities. This allows affected families to enhance their lifestyle based on a person-directed plan.
In this plan, services include:
- Community Support Services
- Respite Care for Family Caregivers
- Employment Assistance
- Alternative Living Arrangements
- Recreational Programs
- Other Waiver Services as deemed
Eligibility requirements:
- There is no age limit.
- You must be determined eligible for Medicaid services before enrolling in the program.
- You must meet specified functional criteria, including having a determination of an intellectual disability made by state law or having been diagnosed by a physician as having a related condition with an IQ of 75 or below.
- You must live in your own home or your family’s home.
- You cannot be enrolled in another Medicaid waiver program.
- Your income and resources may not exceed specified limits.

Every Moment Matters Agency offers Home and Community-Based Services to eligible clients living within our service area scope.
The Home and Community-based Services (HCS) program provides individualized services and support to people with intellectual disabilities who are living with their families, in their own homes, or in other community settings, such as small group homes where no more than four people live. The local authority provides service coordination.
