Hospice focuses on caring, not curing and in most cases care is provided in the patient's home. Hospice care also is provided in freestanding hospice centers, hospitals, nursing homes, and other long-term care facilities.

The hospice team develops a care plan that meets each patient's individual needs for pain management and symptom control.

Among its major responsibilities, the interdisciplinary hospice team:

• Manages the patient’s pain and symptoms; 
• Assists the patient with the emotional psychosocial and spiritual aspects of dying; 
• Provides needed drugs, medical supplies, and equipment; 
• Coaches the family on how to care for the patient; 
• Delivers special services like speech and physical therapy when needed; 
• Makes short-term inpatient care available when pain or symptoms become too difficult to manage at home, or the caregiver needs respite time; and 
• Provides bereavement care and counseling to surviving family and friends.

  Abundant’s team of licensed experts is available for 24-hour care. When symptoms become too difficult to manage, your loved one can benefit from skilled nursing care around the clock.

  In addition to administering prescribed medication, the team is highly trained to offer various therapeutic means of controlling a patient’s pain level and alleviating symptoms.

  Abundant provides medication that are related to your hospice diagnosis.

  Hospice care also provides all necessary medical equipment to ensure your family member is kept comfortable. This includes oxygen, hospital beds, wheelchairs, bandages, adult diapers, latex gloves, and more.

Our Services 4 Levels of Care

Routine Hospice Care
Is the most common level of hospice care. With this type of care, an individual has elected to receive hospice care at their residence, which can include a private residence, assisted living facility, or nursing facility.

General Inpatient Care
Is provided for pain control or other acute symptom management that cannot feasibly be provided in any other setting. General Inpatient Care begins when other efforts to manage symptoms have been ineffective. General Inpatient Care can be provided in a Medicare-certified hospital, hospice inpatient facility, or nursing facility that has a registered nurse available 24 hours a day to provide direct patient care.

Continuous Home Care
Is care provided between 8 and 24 hours a day to manage pain and other acute medical symptoms. Continuous home care services must be predominately nursing care, supplemented with -caregiver and hospice aide services, and are intended to maintain the terminally ill patient at home during a pain or symptom crisis.

Inpatient Respite Care
Is available to provide temporary relief to the patient's primary caregiver. Respite care can be provided in a hospital, hospice facility, or a long-term care facility that has sufficient 24-hour nursing personnel present on all shifts to guarantee that patient's needs are met. Respite care is provided for a maximum of 5 consecutive days.

This ministry visits those of our parish that are homebound and offers them the Eucharist.  

Our homebound ministry does not always mean visiting and bring the Eucharist, it can mean sending a card or baking a treat.

Hospice Care

The patient’s wishes are a primary concern during hospice care. For some, the ability to manage symptoms at home and reduce hospital visits may be a priority. Others may prioritize reducing the burden on family caregivers or remaining in a family home. The goal of Three Oaks Hospice is to always accommodate the wishes of the patient and family.

Hospice Care at Home

In-home hospice care focuses on caring for patients’ needs in their chosen environment. Often, this is the patient’s home, but it doesn’t have to be. For example, a hospice team can provide care to a patient in the homes of family members, an assisted living facility, or a nursing home.

Individuals with renal disease, neurological diseases, dementia, pulmonary disease, heart disease, AIDS, or cancer may qualify for hospice.

Three Oaks Hospice provides tailored care for individuals with these and other conditions.

St. Dominic's is holding monthly educational workshops on mental health. The workshops cover various mental health topics and include guest speakers. There is a Q & A discussion afterward, with a light dinner and beverages served for all.

Workshops are open to the public and are located in the Piper Community Center at St. Dominic Catholic Church.

Hospice is an appropriate course of care for terminally ill patients in the last six months of their lives, at a time when curative treatments are no longer an option.

Hospice caregivers help patients manage their illness and remain as comfortable as possible in their environment. The goal of hospice is symptom management rather than treatment or recovery. Hospice care complements the guidance of a patient’s physician.

Hospice helps ease the burden of families caring for patients during this time.

When is someone ready for Hospice care?

• At the recommendation of their physician
• When diagnosed with a life-limiting illness and when symptoms are no longer responding to a curative treatment
• If you feel that your loved one may qualify for hospice services, please call one of our offices to discuss hospice eligibility and specific health conditions.

Care Locations

Our goal is to provide hospice services in the environment that best meets the needs of the individual patient, caregiver, and other family members. To that end, we provide care in whatever setting our patient calls home:

• Personal Home – When possible and desired, we help our patients remain at home during their terminal illness. • Nursing, Assisted, and Independent Living Facilities – When caring for a patient who resides in a nursing home, an assisted living, or an independent living facility, our hospice team complements the services and skills of the facility staff and works with that staff to care for the patient. Your physician needs to prescribe Primary Home Care (PHC) as this treatment is provided under their supervision.

At VITAS we care for patients with an interdisciplinary team comprising a nurse, physician, aide, social worker, and chaplain. Other clinical professionals, from a music therapist to a trained volunteer, may work with the team to help meet the emotional and spiritual needs of the patient and their family. Our teams design personalized care plans to ensure comfort, dignity, and quality of life.

Most of the time, hospice care is brought to the patient at home, since home is where most seriously ill people want to be: in familiar surroundings with familiar routines and familiar faces.
