HOTBHN’s crisis hotline is managed by ICARE. ICARE provides initial screenings for behavioral health services, crisis assessments, and can refer to other community supports. The Mobile Crisis Outreach Team will be contacted if crisis response is requested.
Mobile Crisis Outreach Team (MCOT):
- Heart of Texas BHN offers 24/7 crisis response services for Bosque, Falls, Freestone, Hill, Limestone, and McLennan counties.
- MCOT provides 24/7 crisis assessments for children, adolescents, and adults experiencing a mental health crisis.
The Mobile Crisis Outreach Team consists of nurses, qualified mental health professionals, and licensed clinical professionals.
MCOT facilitates state hospitalization for those
individuals who meet criteria and can provide information about private hospitals for those insured. Additionally, MCOT will follow up with individuals for up to 90 days to prevent future crisis episodes and connect individuals with community resources.
MCOT provides 24/7 crisis assessments for children, adolescents, and adults experiencing a mental health crisis.
MCOT provides assessments in all community settings including rural and city residential areas, businesses, emergency departments or other health care settings, jails, and schools for individuals who are experiencing a mental health crisis.

Agency Referrals help connect you with local agencies that can provide additional assistance, such as CPS Energy, S.A. Food Bank, S.A. Pet Pantry and S.A. Department of Human Services.
To be eligible for this program you must:
- Be a SAWS residential account holder.
- Meet income requirements.
- Complete the Uplift application, which can be found here.
Here’s how Safe Place works:
Step One – A young person enters a Safe Place and asks for help.
Step Two – The site employee finds a comfortable place for the youth to wait while they call the licensed Safe Place agency. For the greater San Antonio area that is Roy Maas Youth Alternatives (RMYA).
Step Three – An RMYA staff member will talk with the youth to get more information and explain that an SAPD officer can escort them to the RMYA Bridge Emergency shelter. If the child is agreeable, the RMYA staff will contact SAPD to transport the youth to the shelter.
Step Four – Once at the Bridge, direct-care staff and counselors meet with the youth and provide support. Agency staff ensures the youth and their families receive the help and professional services they need.
This program is for youth under the age of 18 in need of immediate help and safety.
Safe Place® is a program of National Safe Place Network (NSPN).

Children Matter is the best option for both biological parents and family members by offering consistent visitation (pending court orders and family plans of service).
Our services include:
Individualized Parent Coaching Each biological parent receives a Visitation Coach as a part of their onboarding. This coach does one on one parent coaching using our twelve-week curriculum called “The Active Parent.” In these twelve weeks, parents will learn skills such as:
- Character development
- Good communication
- Healthy structure
- Positive reinforcement
- Attachment/bonding
- Effective discipline management
- Family life skills
Supervised Family Visitation Children Matter offers our families a safe, comfortable, homelike setting where they can come and spend quality time together in a structured environment for both court-ordered and private services.
Before each visit, the parent receives parent coaching and immediately gets the opportunity to begin putting these skills to practice with their children in the visit that follows.
We work hard to inspire moments of healing, growth, and positivity during each visit as the family goes through the permanency process together.
Case Management We take care of all the details so our clients don’t have to. Our top-notch team members facilitate all communication and visitation scheduling with all parties.

We also provide a detailed report of each visit for the progression of the case and for the safety of the children.

This SA Hope Center program connects individuals and families with Social Workers, whom we call Mentors, to work together and address the root cause(s) of chronic poverty.
Mentors team with the program participants to create personalized goals that increase physical, emotional, and spiritual health by focusing on each person's unique strengths.
When empowering each individual through our programs, they begin positive steps towards achieving their goals of financial literacy, strengthening the family unit, career readiness, adult education, and overall wellness.
This program serves students working towards their GED, ESL, and certification classes such as administrative assistant, CNA, phlebotomy, IT, etc. All Restore Education students are eligible.

Guardian House can help facilitate the exchange of legal documents.
How to Get Started
- Both parties must contact Guardian House and schedule an intake interview to enroll in our services. Staff will NOT contact the other party to schedule an intake interview.
- All paperwork, required documentation and fees from both parties must be submitted before the enrollment process is considered complete.
- After the enrollment process is complete and before visitation begins, a Child Orientation will be conducted with each child participating in supervised visitation services.
- After the Child Orientation is complete, both parties will be contacted by phone to let them know that the non-custodial party may now begin requesting visits.
- In the event that services do not begin, the intake paperwork will only remain on file for six months. After that, the intake interview and intake fee will have to be completed again to have an active intake, or to start services.
Records
Guardian House uses a third party service to provide copies of client records upon receipt of subpoena. Subpoenas must be hand delivered; faxed or emailed subpoenas will not be accepted. The attorney is responsible for paying fees and coordinating delivery of copies with the third party service. Guardian House is not part of this process.
Upon subpoena, the Custodian of Records will testify or make depositions regarding observations made during visits. For court hearings we request that the Custodian of Records be placed on-call, and be notified 30-40 minutes prior to taking the stand.
Fees for Custodian of Records:
Retainer – $750
Preparation & Coordination – $75
On call – $200/day
Court appearance – $100/hour (includes travel time)
A charge of 35% will be added to the total amount if payment is made with less than three business days prior to the appearance of the Custodian of Records.

Methodist Healthcare Ministries' Community Health Workers are certified by the Texas Department of State Health (DSHS).
Services include:
- Establishing positive, trusting relationships with community residents in or from underserved communities.
- Helping patients/clients, families, and underserved communities overcome barriers in language and culture to better connect to quality health care services and resources.
- Connecting underserved populations to health care services and resources.
- Providing informal counseling, support, and follow-up home visits for patients.

Early Head Start and Head Start promote school readiness by encouraging parent involvement and through the cognitive, social, emotional, language, and physical development of children.

As part of our Outreach, Linkage, and Support Services, 210 StreetReach provides referrals, linkage, and support to people in need of substance use treatment, mental health services, medical care, and/or support services.

A partnership with the SAPD Mental Health Unit that pairs clinicians with officers to respond to individuals with Mental Health needs who frequently use 911 and other emergency resources.
Case Managers direct families to many Any Baby Can program as well as help link families to many local and state programs. All Any Baby Can services are offered at no cost - regardless of income - to families who have a child birth through 17 years of age with a chronic illness, disability, developmental delay or health risk and who reside in Bexar County or one of the surrounding 18 counties.

by RAICES - Refugee and Immigrant Center for Education and Legal Services
RAICES established the LEAF fund to ensure that children coming to this country can receive quality legal representation both in detention centers and once they are released. In 2018, our specially-trained team provided ongoing legal counsel to 400 children, and more than 4,500 children received training to help them understand their rights here in the United States.
The LEAF fund is a major source of support for the many RAICES programs benefiting unaccompanied children. Our children’s programs include:
- “Know Your Rights” presentations and legal screenings for thousands of children
- An advocate for children who have had their rights violated
- Legal representation for children both in detention and post-release
- Post-detention care, including ongoing access to counsel for children in their immigration cases
- Assistance with other fees associated with immigration documentation, forms, and applications

If you are currently experiencing homelessness or are at risk of becoming homeless, Homelink can help by assessing your situation and connecting you with the most appropriate housing resource.
After enrolling with Homelink, if your need is not met by an immediate referral, you must maintain your eligibility every 30 days. This will be explained further during your Homelink assessment.
You do NOT need to be a veteran to receive an assessment at this location.
Our Graduate Support Program (GSP) started in 2013-14, and has expanded this year, with the hiring of a Graduate Support Program Director. The GSP offers preparation for our current students to succeed in high school, as well as support for our graduates to excel in high school and college (for a more detailed description of the GSP, see the attached Powerpoint). Each year, 8th graders, and sometimes 6th and 7th graders, receive such preparation.
Such preparation includes:
- HSPT Prep course once a week in the fall semester and into the spring semester, and a College Exploration course in the Spring
- Intensive HSPT workshop offered for free and on-site by St. Anthony High School
- One-on-one guidance for selecting a high school
- Class tours of Catholic high schools and individual shadow days at Catholic high schools
- One-on-one guidance for preparing academically for High School
- High School Panel where SMMS alumni currently attending and teaching in Catholic high school offered insights and advice for our students, as well as fielded questions about their experiences
- High School Fair where local Catholic High Schools visited our school with staff and students, and our 5th-8th graders were able to informally interview and be interviewed
- Help with applications for scholarships, including the Marianist Urban Student Program (MUSP) Scholarship through Central Catholic and the St. Joseph’s Scholarship
- Help with the full range of the application procedure for Catholic High Schools
- The cultivation of a college-bound culture through a College Corner, College dress days, college tours, and our stated goal of college and college signage in the school

NAS is an intensive residential substance abuse treatment program open to pregnant women and mothers of minor children, including those seeking reunification with children in Texas Department of Family and Protective Services custody.
Trauma-informed and gender-specific treatment targets mothers who are struggling with an opioid substance use disorder. Comprehensive case management assists in achieving stable recovery and preservation of the consumer’s family.
