Integrated Community Services (ICS) is our company that provides HCS services (Home and Community-Based Services) in our Abilene, North Houston, and San Antonio service regions in Texas.

This program offers 3-bed homes …offering 3 and 4-person group residences, along with the entire array of available HCS services since 1993 in Abilene and San Antonio since 1993 and more recently in North Houston since 2005. 

Our team understands how to work with people with disabilities while using care, understanding, patience, and encouragement to help our residents excel.

We cater fully to the medical and non-medical needs of our community residents, guardians, caretakers, and partners. From medical services and coordination/training to day habilitation and work placement, our staff is expertly trained to make all aspects of our program comfortable and easy.

The Area Agencies on Aging are dedicated to building a community that supports older residents and allows them to age in place with dignity, security, and enhanced quality of life. Transportation Services

ART provides public transportation bus service to all residents in the service region. Service to and from Bexar County and San Antonio is also provided. ART provides demand response, curb-to-curb transportation service. Door-to-door service may be requested for those customers needing additional mobility assistance.

Sometimes, the best environment for individuals needing support is in their homes or with family. D&S, the local Siveta provider, offers hourly support to help individuals live independently in their home environment. The time spent providing services each week is based on the individual's and their family's needs.

D&S Home and Community-Based Services:

This program is for veterans and unmarried Gold Star spouses whose loved one was killed in action.

Requirements:

  • It would be best if you lived within the particular travel radius of the vehicle to qualify for it.[Check Google Maps for details]
  • Must be a veteran wounded during combat (Purple Heart recipients given priority) or injured during combat training from any American military conflict
    or
  • Must be an unmarried Gold Star Spouse of a service member who was killed in action or killed during combat training
  • Must be honorably retired, separated from the military, or within 90 days of discharge.
  • Must be able to obtain vehicle insurance.
  • Must not have more than one vehicle loan/lease per household.

The YES Waiver is a home and community based services waiver that allows for more flexibility in the funding of intensive community-based services to assist children and adolescents with severe emotional disturbances to live in the community with their families.

Goals:

  • Reducing out-of-home placements and inpatient psychiatric treatment by all child-serving agencies;
  • Providing a more complete continuum of community-based services and supports;
  • Ensuring families have access to parent partners and other flexible non-traditional support services as identified in a family-centered planning process;
  • Preventing entry into the foster care system and relinquishment of parental custody; and
  • Improving the clinical and functional outcomes of children and adolescents.

Family Promise of Greater New Braunfels is a community response to family homelessness. We exist to support, guide and lift families out of homelessness and into safe housing, gainful employment, and independence.

Representatives from local churches have came together to address the need. Family Promise of Greater New Braunfels (FPGNB) is a partnership of churches that open their doors, classrooms, gyms, and hearts to serve as hospitality centers to families experiencing homelessness, partnering with professional case management and other community resources to offer temporary housing, meals, transportation and loving support.

When a family successfully completes our comprehensive shelter program, they have accomplished a series of steps to continue their journey to long-term self-sufficiency. Accomplishments include:

  • A working, realistic budget

The, “Veterans’ Financial Assistance Program” will provide assistance to eligible Veterans, Veterans Dependents and Surviving Spouses.

Through the program, CCSCT will assist beneficiaries with short-term and temporary financial Emergency Services to include: one-time utility payments (may include electric, gas, propane, water, sewer, trash, late fees), one-time rental/mortgage payment (may include rent, mortgage, late fees, application fees, security deposits), and transportation services including bus passes, city and county transit passes, council of government transit service passes, taxis, and ride shares (Uber).

CCSCT will not require beneficiaries to meet a poverty or other income level as a requirement for eligibility. Financial need will be determined by the veteran being behind on rent or mortgage (s) and/or utilities.
