• Have a diagnosis of serious mental illness (SMI)

Have experienced at least ONE of the following:

• Long-Term Hospitalization (indigent and Medicaid eligible population): three or more cumulative or consecutive years in an inpatient psychiatric hospital during the five years before enrollment; or
• Jail Diversion: (active Medicaid population only): two or more psychiatric crises and four or more discharges from correctional facilities during the three years before enrollment; or
• ED Diversion:  (active Medicaid population only): two or more psychiatric crises and fifteen or more total ED visits during the three years before enrollment

No Strings Attached (NSAGirls) is a Survivor-Led Advocacy and Aftercare program under Untethered Ministries, we offer direct care services to youth (13-17) and adult women in transition out of sexual exploitation.

Operation BRAVE at University Health works with community partners to help those living with HIV to learn about the importance of treatment and response.

Our goal is to reduce HIV rates in our community so that everyone can live a Life Unlimited.

Operation BRAVE focuses on community members who are newly diagnosed with HIV, out of HIV care, or in care and not virally suppressed.

Operation BRAVE also focuses on young adults aged 18-29 but serves all ages.

All of our services are 100% confidential and free.

Street Medicine can be thought of as an old-fashioned house call where the roof over someone’s head might be a bridge, a tarp, a tent, the canopy of trees, or the open sky. There are no refrigerators to store medications.  No running water to keep wounds clean or attend to personal hygiene needs. No easy access to transportation, making trips to clinics and pharmacies difficult or impossible.

Due to the austere conditions and realities of street life, street medicine efforts cannot provide comprehensive care to our unsheltered friends; rather, teams aim to build meaningful relationships by caring for acute needs of patients where they live and coordinating more robust care in the community for issues outside the scope of a street medicine practice.

Under the supervision of Medical Director Dr. Hans Bruntmyer, the Street Medicine team spends the majority of their time walking the streets and remote areas of San Antonio and Bexar County equipped with backpacks filled with basic medical supplies, water, socks, and hygiene kits

In addition to predetermined rounds, the team also responds to calls from local outreach workers and law enforcement.

The team does its best to accommodate the environment and effectively care for whatever they might encounter. The team can handle basic primary care issues such as wounds, fractures, sprains, colds, back pain, neck pain, etc. Screenings for blood pressure and blood sugar are also available.

We treat unsheltered individuals.

In order to be eligible for in-home care, a functional need threshold, also referred to as a medical need, must also be met. This could include the client requiring help with mobility, transferring from a bed to a chair, toiletry, bathing, doing laundry, or meal preparation.

Your case manager will send us the referral information, and we will reach out to you to schedule a time to meet you and family members that are part of your care team.

This first visit is performed by a qualified health professional to discuss all aspects of care needed and answer any questions or concerns you may have. We will then make sure to match you with the perfect caregivers and create a care plan to fit your desired schedule.

Services are usually paid directly by the person receiving care (private pay) or through long-term care insurance or Medicaid.
