AccessAbility Fest is a gateway to information, resources, and opportunities for individuals working to overcome barriers caused by physical, cognitive, sensory, and mental health challenges.

AccessAbility Fest is for individuals of all abilities, including those with physical, developmental, sensory and/or mental health challenges, and it connects over 4,500 individuals to resources each year, creating opportunities for disability service organizations to collaborate, make referrals, and provide more comprehensive services.

This program is held in high traffic venues to raise awareness about the value, successes, and potential of people with disabilities among the general public.

The festival is FREE and OPEN for the public to attend.

• Access to equipment, assistance with job searches, and case managers who provide resume templates, complete assessments, and United Way referrals and supports.

Hospital-to-Home Transition
For a medically complex child or adult, the transition from hospital to home can be a difficult one. Our mission is to make this process smoother and less overwhelming by assisting with everything from coordinating the discharge process with the hospital case manager, to settling in after the first time you take your baby home. No matter your situation, having the right help in place can make a world of difference to you and your family.

Our prescribed pediatric extended care services let your kids be kids. Our state-of-the-art facility was designed with children in mind, allowing them to play, dream, and grow — all while meeting their physiological, developmental, nutrition, and social needs. Our goal, through PPEC, is to impact the lives of our medically complex children from the early stages of life all the way into early adulthood.

What is PPEC?
Prescribed pediatric extended care, or PPEC for short, is provided at a non-residential center that serves medically complex or technologically dependent children under the age of 21 who require short, long-term, or intermittent medical care due to medically complex conditions.

The ACP PPEC provides services to medically complex children who cannot attend a regular daycare. Our services meet the child’s physiological, developmental, nutrition, and social needs. We provide nursing and psychosocial services, developmental therapies, caregiver training, and personal care support to our children.

It can be confusing to find help for older adults and people with disabilities. Aging and Disability Resource Centers (ADRCs) are part of the No Wrong Door system, which is designed to streamline public access to long-term services and supports (LTSS).

ADRCs serve as a key point of access to person-centered LTSS specialized information, referral and assistance and provide one-stop access to information for people who need help finding LTSS.

ADRCs help cut through the confusing maze of funding sources, multiple intake systems, and eligibility processes.

The purpose of the IDD Transition Support Team is to provide medical, behavioral, and psychiatric support to: 

• Local Intellectual and Developmental Disability Authorities (LIDDAs)
• Home and Community-based Services (HCS) program providers
• Texas Home Living (TxHML) program providers
• Licensed Mental Health Authority (LLMHA) / Licensed
• Behavioral Health Authority (LBHA) services
• Community Living Assistance Support Services (CLASS)
• Deaf Blind with Multiple Disabilities (DBMD)
• Any other professionals who are working with people with ID/DD
• Our team aims to prevent institutionalization by providing medical, psychiatric, and behavioral support from several disciplines of person-centered care.

The program is open to all older adults with IDD and their caregivers. “Caregivers” can be in a paid or unpaid role and do not have to live with the older adult with IDD. Individuals with IDD and caregivers are welcome to participate whether receiving services from AACOG or not. The program is open to professionals in the field.

• GRASP helps with pharmacy and other emergency assistance in critical situations.
• GRASP assists with budgeting and planning household finances to avoid or manage future crises and stressful economic situations when emergencies arise.
• GRASP offers transportation to seniors and the disabled to attend congregate meals, medical appointments, shopping, social activities, and other special trips.
• GRASP coordinates group activities and programs for seniors at our community center and nutrition site that include congregate noon meals each day.
• GRASP volunteers deliver daily hot lunches to homebound elderly in the GRASP service area.
• GRASP connects those in need of other services with area agencies, churches, and organizations through its referral and information program.
• GRASP offers local youth opportunities for learning and activities at the community center

CHWs in our Connecting Kids to Care initiative provide bilingual information and application assistance for government programs such as Medicaid and CHIP (Children’s Health Insurance Program) to families with children, pregnant women, the elderly with certain incomes, and people with disabilities. Our CHWs are also trained to help families figure out if they qualify and provide guidance on what documents are needed for the application process. Those who qualify receive benefits such as coverage for visits with doctors and dentists, prescription drugs, vaccines, eye exams, hospital care and more.
