Given the choice, most people want to stay in their homes. Sometimes, people need help to remain at home.
In-Home Personal Care Services are for older adults who prefer to stay at home but need ongoing care that cannot easily be provided by their families or friends.
When health issues prevent them from performing simple tasks, impair their mobility, or put their safety and well being at risk, their options may be limited. Some may move in with family members. Some go to nursing homes. Those who do not qualify for public assistance and cannot afford private care often face a serious struggle for independence. That is where Family Service Association steps in to lend a gentle hand.
Older adults who have the ability to remain independent in their own homes with the assistance of supportive services may receive up to 10 hours of direct service each week in the home for personal care and home assistance, including the following:
- Meal planning and preparation
- Light housekeeping,
- Assistance with the daily activities associated with living at home, such as bathing, dressing and grooming, washing and combing hair.
A complete evaluation and assessment is conducted on the first home visit, with on-going supervision at least every 90-days. Follow up visits are scheduled to monitor client needs and to ensure continued appropriateness of services.
An excellent investment, the program can serve 100 clients for less than it would cost to care for 10 patients in an alternate care setting, for the same one-year period.
Services are offered on a sliding fee scale that is based on the client’s ability to pay.
Funding is provided by United Way, Client Fees, and Private Foundations and Corporations.
The San Antonio AIDS Foundation provides a continuum of care to low-income HIV-positive men, women and transgender individuals. Our services include resource and benefits assistance, case management, mental health counseling, medical care, pharmacy services, housing, and a year-round hot meal program. We offer onsite and mobile testing for HIV and STDs, as well as HIV prevention education presentations in local public schools, at colleges and universities, adult probation facilities and teen detention programs.

Our staff specializes in skilled nursing care, long-term care, and rehabilitation services as well as hospice and respite care. Our medical director and nursing staff provide a wide range of health care services for those with multiple needs and diagnoses including but not limited to:
- Closed Head Injury
- Congestive Heart Failure
- COPD
- CVA/Stroke
- Hip Fracture
- IV Therapy
- Ostomy Care
- Peripheral Vascular Disease

The apartments are Housing and Urban Development (HUD) subsidized apartment complexes, designed exclusively for adults with intellectual and other developmental disabilities, and are supervised by the Suzanne Smith Management Company.
The apartments have one or two bedroom options with a community room for gatherings and a laundry room.

Mission Road Developmental Center provides residential care for adults with intellectual and other developmental disabilities through the ICF (Intermediate Care Facility) program and the HCS (Home and Community-Based Services) programs. The Texas Health & Human Services Commission monitor and license the HCS and the ICF group homes and the assisted living program. We have 12 HCS and 4 ICF for a total of 84 residents.
Home
Mission Road has 16 adult community homes throughout the city of San Antonio and one assisted living home on the main Mission Road campus for adults. Residents living in the homes are assisted by well-trained, 24-hour AWAKE staff. During the week, residents spend their day working, training for a job, volunteering or engaging in a social/habilitation program. Evenings are spent acquiring and refining the skills needed to live a more independent and fulfilled life. Weekends are for shopping, completing chores, recreational activities and spending time with friends.

The goal of the program is to provide supportive services to optimize the independence and productivity of the adults in an affordable living arrangement.

Host Home/Family Model/Family Home Providers support and assist individuals with daily living skills and other individual needs in a manner similar to the residential program, however, this service is provided in the caregiver’s personal home.
Living with a family is a good choice for many people with developmental disabilities and can be a rewarding and meaningful experience for all those involved. A warm and caring family environment not only allows more relaxed schedules and increased independence, but also develops close bonds and establishes valuable relationships that will last a lifetime.
D&S is committed to individuals in Host Home/Family Model/Family Home by always providing:
- The utmost respect and courtesy regarding individual needs to create compatible placement options
- Pre-screened and trained Providers selected by the individual or his/her guardian/conservator

Sometimes the best environment for individuals needing support is in their own homes or with family. D&S, the local Siveta provider, offers hourly support to help individuals live as independently as possible in their own home environment. The time spent providing services each week is based on the needs of the individual and their family.
D&S Home and Community-Based Services:
- Transportation to work, appointments and other activities
- Housekeeping
- Dressing and personal hygiene
- Meal preparation and eating
- Medication administration (as permitted)
- Community activities and outings
In addition, D&S provides Respite Care, which allows primary caregivers a break from their normal caregiving duties.

HCS facilities are 3- or 4-bed group homes that are available through the Home and Community Services Medicaid waiver program which is certified and licensed by the state. Individuals must be diagnosed with an intellectual disability and have an IQ of 75 or below with a related condition.
We also serve individuals who live on their own in an apartment setting.
Private pay option: Individuals with developmental disabilities who do not have Medicaid funding can pay privately for services in a 3-bed or 4-bed group home. We also serve individuals who live on their own in apartment settings.
ICF facilities are 6-bed group homes funded by Medicaid. These homes are certified and licensed by the state to provide a full range of training and care to individuals with intellectual and other developmental disabilities.
ICF group homes include:
- 24 hours nursing supports
- Therapy
- Nutritional and dietary supports
- Behavior supports
- Medical and dental supports that are not funded by Medicaid
- Active training to master new skills
This program is for individuals with intellectual and other developmental disabilities (IDD).
All services are paid by Medicaid and personal income is managed by the provider, representative payee, or the client themselves. If the client works, a percentage of earnings is refunded to Medicaid. SSDI (Social Security Disability Income) funds can continue with a percentage repaid Medicaid.
RMI's Semi-Independent Apartment program gives adults with intellectual and developmental disabilities the ability to live in the community with more freedom and more responsibility. Residents in this program lease their own apartment and pay their own expenses. They may choose to live by themselves or share an apartment with a roommate.
At San Mateo Apartment Homes, Waters at Elm Creek and other apartment communities in San Antonio, RMI residents work, play, shop and worship with others in their neighborhoods. With ongoing support from RMI staff, residents are able to meet the everyday challenges of living independently.
Adults with developmental disabilities can choose to live in a foster home with a family member or other individual as their foster care provider.
RMI contracts with the foster care provider to assist with daily living skills, personal care, transportation, and other activities that allow the client to live as independently as possible in a safe and supportive environment.

Funding for foster care is covered by the Home and Community-Based Services (HSC) Medicaid program. Individuals who do not qualify or are waiting for HCS funding may pay privately to live in a foster home.

The Southwind City Locals program is designed for men and women of a wide variety of abilities who desire to live independently within the community and have the necessary skill set to do so with minor assistance.
City Locals live in our partnered, affordably priced apartment complex located on the Northwest side of San Antonio. Here, they maintain their own lease agreements, while enjoying the fellowship and community of living alongside other Southwind City Locals.
City Locals have the opportunity to attend the Southwind Day Program, should they so desire, where they may participate in community impact team mission work and attend Life Groups and Worship Gatherings.
Other aspects of the City Locals program include:

- City Local Program Fee: $300/month (organic, locally sourced, meals will be available for purchase at the Southwind Social Club Monday-Friday for $5.00/meal). Your monthly fee covers all of the services listed above.
- Rent: Apartment application fees and deposits are typically around $150). Monthly rent is approximately $445 per person for a two-bedroom apartment and $690/per person for a one-bedroom (Locals opting to live in a one-bedroom must pass a screening for living alone before applying).
- Utility Cost: Approximate cost of electricity for a two-bedroom apartment ranges from $40-$50 per person each month, based on the Local’s combined use. Internet is approximately $60/month. Trash and water are included with rent.
- Groceries and Entertainment: Most locals budget $80/week, on average for groceries and fun. Locals opting to use ride-hail services rather than riding the bus or utilizing VIA Trans to get to work or go out should budget an additional $30/week.
