The San Antonio AIDS Foundation provides a continuum of care to low-income HIV-positive men, women and transgender individuals. Our services include resource and benefits assistance, case management, mental health counseling, medical care, pharmacy services, housing, and a year-round hot meal program. We offer onsite and mobile testing for HIV and STDs, as well as HIV prevention education presentations in local public schools, at colleges and universities, adult probation facilities and teen detention programs.

Activities to help intellectually disabled individuals become more independent in daily and personal skills.
This program assists with acquiring, retaining, or improving self-help, socialization, and adaptive skills necessary to live successfully in the community and to participate in home and community life.
Sisters Care In-Home Care provides caregivers in an adult's residence to assist them with bathing, dressing, grooming, meal preparation, organization, and companionship.
Services offered are paid through private pay, long-term care insurance, and A.A.C.O.G. funding.
Additionally, Sisters Care In-Home Care has a non-profit program called Project Elder Help that offers caregivers on a reduced fee basis if approved based on application and grant funds available.
Our center is designed to keep individuals living in their homes while relieving the caregivers of daytime duties and ensuring that their loved ones still receive proper care in a safe and friendly environment. This cost-effective long-term care alternative also allows both families and those we serve the opportunity to maintain normal daily schedules.
Our team includes an RN consultant, LVN, nutritionist, dietitian, and cook.
Our clients are fed breakfast, lunch, and a snack; socialize; and take part in activities that include bingo, arts and crafts, exercise, and more.

APS investigates abuse, neglect, and exploitation of adults who are elderly or have disabilities and who live in the community. Any adult who has a disability or who is age 65 or older that is in a state of abuse, neglect, or exploitation may be eligible to receive adult protective services.
Victims of abuse, neglect, or exploitation may get short-term help with shelter, home repairs, food, transportation, managing money, medical care, home healthcare services, and mental health services.

Mission Road Developmental Center provides residential care for adults with intellectual and other developmental disabilities through the ICF (Intermediate Care Facility) program and the HCS (Home and Community-Based Services) programs. The Texas Health & Human Services Commission monitor and license the HCS and the ICF group homes and the assisted living program. We have 12 HCS and 4 ICF for a total of 84 residents.
Home
Mission Road has 16 adult community homes throughout the city of San Antonio and one assisted living home on the main Mission Road campus for adults. Residents living in the homes are assisted by well-trained, 24-hour AWAKE staff. During the week, residents spend their day working, training for a job, volunteering or engaging in a social/habilitation program. Evenings are spent acquiring and refining the skills needed to live a more independent and fulfilled life. Weekends are for shopping, completing chores, recreational activities and spending time with friends.
Apartments
Mission Road Ministries sponsors a supervised apartment program for adults with intellectual and other developmental disabilities who need minimal supports. The goal of the program is to provide supportive services to optimize the independence and productivity of the adults in an affordable living arrangement.
Mission Road has three apartment complexes in San Antonio that provide affordable housing and services for the tenants. Suzanne Smith Management Company supervises the three apartments: Independence Square, Murray Manor and Meadow Brook. The apartments are Housing and Urban Development (HUD) subsidized apartment complexes designed exclusively for adults with intellectual and other developmental disabilities. The apartments have one or two bedroom options with a community room for gatherings and a laundry room.

Our centers have an open space setting for wheelchair-bound individuals to move around. We provide a clean, safe, and comfortable environment for our clients to spend the day.
Services include:
- Transportation. We arrange for our clients to have safe transportation to and from their homes daily, making adult care services convenient. We hold Field Trips once in a while, enjoy visiting local libraries, local shops, and parks with our staff members accompanying a small group of clients. We keep our daily activities fresh every day. Boredom is a thing of the past.
- Therapeutic Activities. Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our adult day care can assist you with daily tasks such as mobility, eating, exercising, and grooming.
- Healthy Meals. Our centers serve breakfast, lunch, and a mid-afternoon snack prepared in our own central kitchen. A registered dietitian designs our menus, ensuring that all dietary needs are met.
This program is for adults (18+) with intellectual and development disabilities (IDD).
We contract with Amerigroup Health Care, Molina Health Care, Superior Health Plan, AACOG, UnitedStates Veterans ( VA ).
If a client is eligible under the Medicaid Program, the services will be paid in full by Medicaid and will be at no cost to the client.

Through a wide variety of activities and services, we have developed a proven series of programs that will provide the most comprehensive and positive environment for our clients. We provide clean, safe and stimulating areas designed to instill a sense of comfort and peacefulness, while educating and counseling those with special needs and giving them chances to succeed in many facets of everyday life.
Community Volunteer Opportunities
Making a strong connection with the community is very important for developmentally challenged individuals. Volunteering in community projects helps to strengthen those connections. These volunteering opportunities helps our folks build a stronger sense of self-worth. For this reason Stepping Stones makes every effort to locate volunteer opportunities for our people.
Job Coaching
Some of our folks have jobs when they join our program and some are wanting to find jobs. We offer job training to help those with jobs improve their skills to make it easier for them to keep working. And we offer assistance for those looking for work. As a team the staff frequently meet to assess our folks' job performances and then offer suggestions on how to help them improve.
Health Coaching
When health issues make it necessary for special needs people to make lifestyle changes it can be frightening and confusing. We, at Stepping Stones Day Hab, help guide them through the confusion by providing an on site Certified Integrative Nutrition Coach. Our Health Coach works with the individual’s team of health care professionals developing a plan of action that will address their health concerns. Once this plan is in place the Health Coach directs them on how to follow it.
This program is for adults with intellectual or developmental disorders (IDD).

HCS facilities are 3- or 4-bed group homes that are available through the Home and Community Services Medicaid waiver program which is certified and licensed by the state. Individuals must be diagnosed with an intellectual disability and have an IQ of 75 or below with a related condition.
We also serve individuals who live on their own in an apartment setting.
Private pay option: Individuals with developmental disabilities who do not have Medicaid funding can pay privately for services in a 3-bed or 4-bed group home. We also serve individuals who live on their own in apartment settings.
Angel Care Center provides adult daycare that includes ADL classes, activities and outings.
