San Antonio Community Resource Directory

The SA Kids BREATHE (SAKB) program is a free city program whose primary goal is to improve the quality of the lives of children with asthma by teaching them and their families best practices to control their asthma.

The SAKB program will help keep kids out of the hospital, keep kids in school, keep parents at work, and save families and healthcare systems money.

SAKB Community Health Workers meet with these families at their homes (virtual or in-person) to improve asthma control. Education and support are provided over a 12-month period.

  • 5 to 6 visits over a 6-month period
  • 3-4 at-home visits
  • 1 visit with a healthcare provider
  • 1 visit with a school or daycare

Check-ins with the family are provided over an additional 6 months if needed.

To be eligible, children must meet these three requirements:

  • Age 3-17
  • Diagnosed with asthma
  • Residing within one of the City of San Antonio's 10 Council Districts. (Not sure? Click here and type your street address in the search box.)

Anyone can refer to the program. Call or email to get started.
