MHP Salud (Nonprofit )

MHP Salud serves communities by embracing the strengths and experiences of individuals and families, engaging them to achieve health and well-being.

Our health outcome program consists of several community health worker-led initiatives that target various health and social issues such as access to health care, chronic disease control and management, healthy relationships, aging, nutrition, and more.

In addition, MHP Salud is dedicated to supporting organizations in their efforts to improve the productivity and performance of their new or existing CHW programs. We continuously collaborate with national health organizations and associations to develop training and resources and provide technical assistance centered around the CHW profession.

As individuals age, services like health education, case management, and application assistance become vital for those who want to maintain their health. CHWs are equipped to provide these services and can outreach in the places where individuals live, work, and play at every stage of life.

Our approach to supporting survivors of Intimate Partner Violence is built around support group sessions that combine creative self-expression, emotional empowerment, and education.

Healthy Living

MHP Salud’s Healthy Living initiative provides accessible support and education to community residents so that they can live healthier lives.

Connect to Care

MHP Salud’s Connect and Care Initiative aims to improve health care access for underserved community members by providing information on health care services and offering application assistance to available social programs.

Our Community Health Worker (CHW) Navigators are trained and certified by the Centers for Medicaid and Medicare Services (CMS) to help you enroll in a health insurance plan.

At no cost to you, we are ready to help with, enrolling in health insurance, vision, and dental plans and help you understand available health coverage options including Medicaid, CHIP, STAR+PLUS, and more.

We offer FREE virtual and in-person appointments at various locations in these counties:
