Challenging behavior can often interfere with an individual's ability to achieve self-determination and independence. Our team uses person-centered thinking and a positive behavior approach so that our treatment plans focus not only on the challenging behavior, but on increasing the individual’s ability to access, control, and expect the routines and activities that are most important to them.

Intervention and Treatment - If an individual behaves in a way that is problematic for themselves or others, a clinician from our team performs a functional behavior assessment that carefully identifies the situations and environments where problematic behaviors are most likely to occur. Our clinicians not only spend several hours with the individual and review their records thoroughly but also use a collaborative approach by collecting information from the people who know the individual best, such as their family members and guardians.

Based on these findings, the clinician identifies skill areas that the individual may benefit from developing, and trains staff on how best to prevent and respond to situations most likely to lead to challenging behavioral issues. The clinician continues to monitor their progress, attend planning meetings with staff who work with the individual, and adjust plans and techniques according to the individual’s progress and improvement.

Because of our in-house team and the support they provide, we’ve successfully decreased the number of challenging behaviors and psychiatric hospitalizations for individuals in our program.

At D&S Community Services, we believe all individuals deserve daily opportunities to maximize their potential and learn skills that promote independence in their community. D&S Day Habilitation programs in Texas, Tennessee, and Kentucky provide community integration, employment training, and other meaningful and productive activities in various settings. Activities are tailored to align with each individual’s plan that builds on their strengths, talents, and skills.

Independent Living Training
The D&S Day Habilitation Program provides skills training classes and other opportunities for individuals to improve their independence in the community. Skills training classes to improve activities of daily living include:

• Personal grooming and hygiene
• Money management
• Reading and language
• Proper nutrition and exercise
• Meal preparation and cooking
• Use of public transportation
• Safety
• Communication
• Etiquette
• Employment Training

The Day Habilitation programs work with the Supported Employment program to train individuals for job readiness. Training may include opportunities to work on a supervised lawn care enclave, perform piecework assembly tasks, or participate in janitorial training at the Day Habilitation facilities. Each activity and experience is used to help prepare individuals to work in the community.

Social Skill Development
The D&S Day Habilitation Program sponsors numerous social events to help individuals develop appropriate social skills and learn to interact better with others. Individuals actively participate in the planning, setting, and promoting of each event.

• Planned outings to local parks, recreation centers, movie theaters, and libraries

Many individuals need different outlets to express themselves. We like to foster these needs through various activities that focus on self-expression. These activities include:

People with intellectual and developmental disabilities must manage healthcare challenges that range from routine minor illnesses everyone experiences to chronic diseases that occur with aging to unique medical situations specific to a disability.

D&S Community Services, the local Siveta provider, is committed to partnering with each individual to achieve and maintain the level of health each person chooses. In addition to managing routine minor illnesses, the nurses and support staff with D&S are experienced in providing excellence in care for asthma, diabetes, osteoporosis, seizures, hypertension, and Alzheimer’s.

Our employees are also skilled in preparing and serving nutritious meals that meet a person’s needs for thickened liquids, pureed or modified diets, or eating with special utensils. Attentive personal care is also provided to help with bathing, dressing, homemaking, and other activities that allow people to live in the community of their choice.

Host Home/Family Model/Family Home Providers support and assist individuals with daily living skills and other individual needs like the residential program; however, this service is provided in the caregiver’s home.

Living with a family is a good choice for many people with developmental disabilities and can be a rewarding and meaningful experience for all those involved. A warm and caring family environment allows more relaxed schedules and increased independence, develops close bonds, and establishes valuable relationships that will last a lifetime.

• The utmost respect and courtesy regarding individual needs to create compatible placement options
• Pre-screened and trained Providers selected by the individual or their guardian/conservator
• A dedicated team of local professionals to provide guidance and facilitate the provision of all support needed
• Scheduled activities for individuals and their host families

D&S partners with independent contractors to provide specialized therapies to individuals who require additional technical services.

Services include occupational and physical therapy, speech and language pathology, orientation and mobility, and behavior analysis services.

Sometimes, the best environment for individuals needing support is in their homes or with family. D&S, the local Siveta provider, offers hourly support to help individuals live independently in their home environment. The time spent providing services each week is based on the individual's and their family's needs.

D&S Home and Community-Based Services:
