CHDI’s mission is to continuously improve the health and well-being of the community we serve. This includes individuals without any form of health insurance, the underinsured, and individuals who receive public insurance.

Recent increases in the number of families living in poverty and the number of elderly unable to afford comprehensive care have created an excessive demand for quality and affordable health care. For thousands of residents in our tri-county service area, CHDI is the primary source of health care.

CHDI’s service delivery model encompasses a philosophy that views the community’s resources and needs as the primary concern in planning and operating all agency activities.

CHDI is dedicated to serving the well-being of its clients without regard to race, sex, color, religion, national origin, disability, or ability to pay for services.

Community Health Workers, or Promotores de Salud, increase wellness by reaching out to, connecting with, and engaging underserved families and communities to help improve health outcomes for themselves, their families, and the communities in which they live.

Methodist Healthcare Ministries' Community Health Workers are certified by the Texas Department of State Health (DSHS).

Services include:

  • Establishing positive, trusting relationships with community residents in or from underserved communities.
  • Helping patients/clients, families, and underserved communities overcome barriers in language and culture to better connect to quality health care services and resources.
  • Connecting underserved populations to health care services and resources.
  • Providing informal counseling, support, and follow-up home visits for patients/clients.
  • Aiding in the creation and maintenance of community partnerships, collaborations, programs, and services.

Our mental health services focus on the prevention, screening, assessment, and treatment of mental disorders and behavioral conditions. We aim to improve health and quality of life for people affected by these conditions. In addition, mental health and physical health are closely connected. Mental disorders like depression and anxiety can affect a person’s ability to take part in healthy behaviors. Similarly, physical health problems can make it harder for people to get treatment for mental disorders. That is why our medical and mental/behavioral health teams work closely together to improve our patient’s overall health and well-being. We care for the entire person.
